My guess would be that you have unchecked the "announce when caps lock key is pressed" checkbox in the VoiceOver preferences. Are you getting other sounds from VoiceOver? Other sounds would be things like progress clicks and the different clicks when webpages load and stuff like that. So under the "sounds" tab in the VoiceOver preferences you can find the checkbox for muting voiceover sounds. Make sure that's unchecked.
