I have the same issue with my AV receiver. In my case, I know it’s because of the sound “sleeping” when there’s no input. Voiceover doesn’t generate enough sound to keep it alive. If there’s a setting to change this behavior there’s nothing in the manual to indicate it. I doubt there’s enough interest to get anyone to do this, but I wonder if an app could be developed that would just produce a sustained carrier that would keep units like this from cutting out, or maybe a better approach would be to see if Apple might consider building something like this into Voiceover directly. It seems enough people are experiencing it to make it worth pursuing. In the meantime, the only solution I have found besides just doing a lot of flicking until I generate enough chatter to get the thing to come back, is to hook up a bluetooth speaker or headset and then turn it off once you have what you want going. Another trick is to start music playing. Far from optimal but it does keep VO from cutting out.
