I had an issue today while trying ti install third party printer software. The install window would go busy while I tried to get focus on the security agent that has no window. It never shows up in the Window Chooser and always hides behind the window that is busy.

Also, I highly doubt that resetting the SMC / PRAM would make any difference at all.

Try turning off VO, then press cmd-tab to take you out of Safari, then turn VO back on and cmd-tab back to Safari.  See it it behaves better after VO has been restarted.  Sometimes the VO buffer gets cranky.
