It is tricky to get right, especially at first.

Firstly lets say that you have your VO cursor at the beginning of a sentence. 
Use the two finger twisting gesture, i.e.,   the rota so that you have it on 
words.
Now put two of your fingers close together on the screen and move them apart.  
You will hear VO say the first word of the sentence and then say selected. If 
you keep moving your fingers apart you will hear VO announce the next word and 
that it is selected. The most important thing to remember when performing this 
pinch out gesture is to ensure that you move your fingers directly apart from 
each other without any twisting. If you don't do this, VO can misunderstand the 
gesture and interpret it to be a rota gesture. 

I hope this helps, just keep practicing. As I mentioned this is not the easiest 
of gestures to master. I think that as VO users we are at a slight disadvantage 
to our non VO counterparts as they do not have the rota gesture available. This 
means that they don't need to be as careful to ensure the outward or inward 
movement is as linear as we do when performing the pinch.
