There appears to be a major bug in OS X 10.7.3 and OS X 10.7.4. VoiceOver users 
cannot create keyboard shortcuts, other than for specific applications, in 
System Preferences.

I really hope that you can help with this one.

To reproduce this bug, Turn VoiceOver on and open System Preferences.
Go to the Keyboard pane and select Keyboard Shortcuts.
Select a category in the first table, then navigate to the second table and 
interact with it.
Choose a command for which you wish to create a shortcut (it must be one that 
doesn't have a shortcut already).
For this purpose, I've chosen from the Services category, Open Selected File in 
TextEdit.
Interact with the second column which contains the command name, the add 
shortcut button, and a text field.
Whether the first column check box is checked or not, the command name and text 
field in the second column are dimmed.
Try clicking the button. I tried VO-space, Space on its own, VO-Shift-Space, 
Return, bringing the mouse, turning off VO and double-clicking with the 
trackpad, all to no avail.
