Jessica, My initial opinion of the MainStage app was the same - totally inaccessible. When I opened the app, I just got a completely inaccessible screen, and VO wouldn't say a thing, I couldn't even close the window easily.
Then a sighted friend read the screen and told me that it was a download message saying that MainStage was downloading essential content, and that it might take up to a few hours. So, I waited, and in a few hours, it had loaded up the initial 2 or 3 gigabytes of sounds and the app opened up. Again, an inaccessible screen. This was a splash screen showing how to connect an input device. So, I connected an input device (a midi keyboard) and main stage opened up and was fairly accessible. I was then able to download the remaining content including all the GarageBand JamPacks (an additional 15 GB). So, MainStage is in fact fairly accessible, it's just that initial download screen and the input device connection splash screen that's a VO deadzone. And, once you plug in a keyboard or guitar, (That may not even be necessary), the app is VO accessible. I haven't messed around in MainStage since its initial opening, but at some point I'll read the help files and check it out.
