I've had the same issue on both my 2011 iMac running Mountain Lion and 2011 MacBook Air now running Mavericks. Like Tim, I've always solved the problem by turning the TrackPad commander off, and physically clicking it once. This usually brings VO focus back and things work fine. Other times I just type my password and press return, and then I'm logged in and VO starts working again. Very occasionally I just toggle VO on and off with command-F5, but the other methods are more reliable. Cheers, Nic
