I'm not sure what to say,
the number you're getting from "iwlist ath0 txpower" should be the max
(19dBm) you can set per the regulation of the card and the regional
domain.
If its a 400mW card, maybe its internally fudging the numbers like here:

You might have to test the card through an actual power analyzer to see
the signal power its generating.
