Hi Aaron, Our sysadmin found the problem. A fix was released for bug #1576643 regarding a similar problem (https://bugs.launchpad.net/mahara/+bug/1576643) but related only to iPad.
We are running Mahara 16.04.1, fix was released in 16.04.2.
