Very low volume senders can have problems because they have limited data to build reputation statistics on.
Emails consisting of almost no text with an attachment are also a bit suspicious so could be junked by the recipient, and, along with the very low volume, that could trigger blocking.
I don't know what the text is, but could you make it more personalised, so the recipient can be more certain it's not phishing?
