In my opinion, a fake.
*Rivoalti* as the name of Venice on a map of the Venetian lagoon of the 13th century? Bah... I don't believe it.

Rialto-Rivolati was the initial name of the main city of the lagoon and the initial name of the sestiere of San Marco (Civitas Rivoalti). The De Administrando (10th cent.) uses the form Rivalto while Venetia seems to designate the whole province (for instance "/apofevgein o laos pros Venetian/", DAI 28, line 44). In other Greek sources (Alexiad) it is not clear if Venetia stands for the city only or the whole province. Add to that, the fact that Constantinople was considering Venice as a part of the Empire until, I would say, 11th cent. maybe even later and that scholars were using older material when depicting or drawing remote lands. Moreover, Diehl (La République de Venise) uses the form Rialto until AD 1000 and Braunstein-Delort (Venise portrait historique.., Paris 1971) are citing a legal document of 1179 made " in Rialto". Since these facts are scarcely known, if the document was a fake "Venetia" would be used instead of "Rivoalti".

We should examine 2 points:
- the cartographer is named "Theophanes, a Greek" not "Theophanes THE Greek"
- the date: XHHE. What is its exact transcription in Western arab numerals? Which calendar: Anno Domini or Anno Mundi?
