To whoever,

I'm currently redesigning a wiki I operate (redesigning it in the Foreground 
skin) and I'm looking to  do something unique, which is:  allow end users to 
create forms. Right now I allow embedded forms from Google, Jotform, Wufoo and 
SurveyMonkey (for those wondering how -- google & Survey monkey via the Widgets 
extension & the other two from the Widgetframework extension), but I would like 
to allow users the ability to use formatted  templates inside the Wiki to 
create those forms. Examples of forms I would like are: polls, surveys, order 
forms, etc. 

I could design these forms via the secureHTML extension, but that wouldn't 
allow the flexibility of allowing users to change the questions, etc. Anyways 
does anyone have any notions on how to do this? 

On another note, can SOMEONE connected with Mediawiki.org fix the Extension 
Matrix? After all it's only been October 2013 that it hasn't worked.
