Thanks, but Semantic Forms is not what I was referring to. Semantic forms, 
which I use, is for editing a Mediawiki page, and the template calls in that 
page via a Form -- what I was looking for is an easy way to allow page editors 
to add forms embedded into a page. Those forms could be in turn filled out by 
the end users (readers) without editing the page. A contact email page would be 
the clearest and easiest to understand example. 

Anyone else have any notions on how this could be done? Thanks
