The questy captcha like all other Captchas gets figured out by the spammers. 
But the link I send earlier explains how
to create dynamic questy questions. Meaning the question's answer changes on 
every sign up.

I have a very, very large wiki that was getting spammed like crazy before I 
made the questions 
dynamic. Now I very seldom get bothered.
