Hi, My facility is in the middle of revising
our plans of care. We would like to make the Care Plans more user friendly and
appropriate. Currently we have several interventions on many problems that are
not being used by the staff to document patient care. Is there anyone
that may offer advice or give an example of what your facilities Plans of Care
