I don't know what the Meditech recommendation is; but we went with
the following:
1st 3 of last name followed by 1st 3 of first name followed by two digits
(sequential).
Example: Dr. John P. Smith = SMIJOH00
Dr. Johnna P. Smithers = SMIJOH01
Get the idea?
