Wondering if hospitals could comment on how they handle frequently used 1/2 tabs. Is a separate pharmacy mnemonic created? Or, do you use the manufacture's size and then enter the correct dose. Also, what is loaded in the dispensing machines? The 1/2 tabs, or the full tab and nurses break the tab? Examples: Coumadin 0.5 MG (Is an entry created for the 1/2 MG (1/2 tablet) and then loaded into Pyxis/Omni), or is the 1 MG mnemonic used with a dose of 0.5 and the nurse breaks the tablet.
