Good Morning, One of our hospitals is looking to re-design the Plan of Care. We would like to know how other hospitals are documenting a Plan of Care. Are you using the Plan of Care routine? If so, how specific are your Diagnosis (and how many do you have)? If you do not use the Plan of Care Routine, how do you document and define Goals/Outcomes? Your help is appreciated as always!
