Why would you want to limit it to once per stay? If it truly is important, it should be important to perform it with each creatinine (speaking based on my previous experience as a laboratory Director). I know many physicians are using it as a basis for increased reimbursement on Medicare patients for their outpatient billing, but that is a poor excuse for ordering it if the result is not going to be used for clinical treatment decisions. In my previous position, we reported it with all creatinines run on adult patients.
