April, at our site, the individual department managers initiate the
paperwork for a new (or terminating, or changing) user. This leads to
some inconsistency of timing; we have some managers who are very good
about sending the request in time, others who don't get it until after
the new employee has started.

We use an Outlook form, so it's online, not paper. This has the
advantage of being quick to fill out, and then everyone (HR, IS, other
departments) are all notified simultaneously.

In IS, we almost always accept the manager's request, and provide the
requested access. In some cases, we may question a manager as to why a
certain user needs certain access, or conversely, we may point out to
the manager that the user needs certain access that's not requested on
the form. But most of the time, we provide the access as requested.

It works pretty well, the only issue being the managers who are
perpetually late on their requests. Good luck!

We are trying to streamline our process of creating system access for
our new users.  We have several questions.

1.  Who initiates the new user requests (HR, Departments, Training
Department, etc) for Meditech, Network, other software systems supported
in IS?
2.  Do you use paper or electronic format for requests?
3.  What is your approval process to grant system access?

Any input would be greatly appreciated.  We want make the process less
complicated for all those involved.
