Pam,
I think a lot of us are in the same situation.  The ELF vendor will no
longer offer the chips and do not know of another company who does.  Our
Meditech specialist had no suggestions, but did agree that an E-form
would be a very hard thing to do.  At this point, we are going to test
the laser forms and hope that is satisfactory or another solution is
found.  Maybe some good ideas will post on the L!
