I've seen that approach used, of having two provider entries. Seems to work from many aspects, as long as it is decidedly easy to figure out which one to use. Training of admissions, ancillary, and lab personnel as to when to use which is the greatest challenge.

First, she is a credentialed staff physician with her own private practice. Second, she is a contracted physician for our ED through an outside company.
