Our policy is:
If an employee has forgotten their badge, they are supposed to go home and get 
it. If the barcode on the badge for some reason won't scan, the employee is to 
take the badge to the Pharmacy (after hours). The pharmacist verifies that the 
badge won't scan and will give the user a "dummy badge" to use until they can 
get a replacement. Then the pharmacist will call the IT on call person to 
associate the dummy badge to the user. During business hours, the user will 
call the IT department to fix the problem.

It sound the idea of a duplicate might be good on the surface, but might 
lead to
workarounds (such as nurses carrying copies of medication barcodes).  When 
we do
pharmacist verification of medications that techs fill, we have the ability 
to
use our computer key instead of a bar code on a badge.

I am not certain that this would work at you site, just a thought.
