Feel around the edge of the pressure plate to check if the clutch plate is
central.  The difference in diameter between them should feel the same all
around the pressure plate.  Pretty trial and error I know, but it just
worked for me on my Mazda.
Check the gearbox is lining up to the locating dowels surrounding the bolt
holes at the horizontal position on the gearbox. Finally, check that the box
is going on squarely by checking the gap between the rear of the engine
block and all around the front of the bell housing.  Just a small amount out
and its impossible to get them to slip together.

Oh yes, check that you have the right length clutch throw out bearing sleeve
fitted.  If its a long one and you are using a thick pressure plate you will
have this problem because the throwout race will be trying to throw the
pressure plate.  This will be  followed by a box full of neutrals if you can
close up the gap, as the pressure plate will be fully thrown without pushing
the clutch pedal.
