The lamp out module is in the relay box, behind the fuses. You have
to remove the plastic cover to get to it.
Two failures, either the solder in the base wires (or the "sockets"
pushing down away from the pins on the module) or cracked solder in
the lamp out module itself. You can pry the cover off and re-solder
most times.
