"Standard" tire rotation only applied to bias ply tires, radials have always been rotated front to rear only. They really don't like to have rotational direction changed, it tends to make the steel belts come out.

Front to rear is fine, you won't get one-sided wear on a modern car unless there is something wrong with the suspension. Front to rear will keep more tread on the front tires (they wear faster on the sides from cornering).

I've never completely worn out a tire in normal use, I've always had to replace them due to abnormal wear (bad tie rods or something like that) or belt separation.
