Sounds like it's not finding the menu bar at all.  Make sure that the
group that you're using as a menu bar is available when your stack
opens (i.e., is in that stack or one of its substacks).

As for putting that item in the right place, you just need to make
sure that the "Help" menu is the last button in the group, that the
last item in that menu is "About yourapp...", and that there is a
divider (a line with - on it) just above that line.
