There isn't a patch readily available as there hasn't been much demand
for them. I'll try to make some patches available tonight.

As Emile pointed out, you can get your existing PHP setup with Midgard
extensions by copying the config.status file from your old PHP tree to
the Midgard-PHP tree. Add the --with-midgard option to the file and run
to recreate your configuration.
