[midgard] Can't seem to add a user to Midgard

Can't seem to add a user to Midgard.
I go to Group admin and try and add a user.
After filling in a User form I hit submit, a page entitled "Person
Admin" comes up with no useful info except the Midgard decorations and
the page titled "Person Admin".  Then I go back to group admin page and
the user is not in the database.

Any ideas?
