I am experiencing the same problem.
When I set my loglevel to debug, I get:

So it seems that the http server and the database are
talking and it even tries to serve a page from the
database, but , alas, I still get: 'The requested URL /
was not found on this server'

You also asked what: 'select * from page where id=1;'
yields. Well, for me it yields what looks like an html
page(Midgard Admin) in mysql client.

I have installed from the Monster Package 1.2.5 on Redhat
6.0 i386
