Your config files are OK, although the access.conf is scrambled. This

The problem is related to this. The RPMs are built on apache 1.3.6
on RH60. I have seen success reports for 61 but it may very
well be that modules built for 1.3.6 will not work with 1.3.9
(which is what you have).
Would you mind grabbing the source RPMs for mod_midgard and midgard-php?
They can be installed by doing
# rpm --rebuild mod_midgard-1.2.5*src.rpm
# rpm --rebuild midgard-php-1.2.5*src.rpm
