The MidgardRootFile directive in httpd.conf must define the path to the
file midgard-root.php3. If the path is wrong, Midgard will not work.
midgard-root.php3 should not be in your documentroot.

There can me several reasons for the 'URL not found'.
The errorlog from apache will tell you more, specifically if you specify
LogLevel debug
in httpd.conf
