Add reason for closing an account

When a client account is closed, the user can select one of five options. Same
for groups. These should be added as an option in the template. May not
require a new column, but rather, maybe add to the status column and just make
five options for closed, one for each?
IE
Closed-Blacklisted
Closed-Duplicate
Closed-Other
...
