As an island, I can "create" a pretty good world around myself (A more powerful entity could probably do a better job). I suspect the people that have the bomb shelters installed have neglected to add a dentist to their stockpile of soups and non perishables.
Wood burning seems like a good clean replenish-able natural resource.
