Hi Agata, I have a solution for you. :) Your mhd file is recognized as segmentation / binary image by MITK. This is indicated in the data manager through a red icon instead of the standard gray icon. The "Surface to Image" utility doesn't let you select segmentations. You have to open the properties view (sheet of paper and magnifier icon) and uncheck the "binary" property of your image. Now you can select the image as you would expect it.

In my case the button for converting is never disabled if I have both image volume and surface mesh loaded.
