It will, from the sounds of it, probably be necessary to give up a small amount of the detail. Better that than square artifacts, no? Try running the data thru "yuvdenoise -l 1" before going to the encoder. Perhaps that will help. Did you try using the -N option? A small value might help, perhaps something like "-N 0.6" would be useful. Using the alternate quanization matrices might also be of value - perhaps they would reduce the block artifacts.
