If it was the SMTPDS I would say “Mail Loop” but I don’t think that effects the receiver service like that.
If you run tcpview (sysinternals.com) do you have a bunch of connections open to the service from the net? If so could be a DOS attack
If not I would just restart it and watch it.

Anyone having issues with SMTPRS.exe causing 99% proc utilization?
I dont think its Load issue since im running modus L on a p2.4 with 512 meg ram and 7200RPM 8MB cache drive with maybe 500 accounts.
