This "hating to read" business in the upper grades is very noticeable to some of us, too. We've had some success with locating "just right books" for these reluctant readers. The local Unitarian Church has been supporting us financially to purchase paperbacks that fit the interests of some of our struggling readers. Also, we've found that sometimes an adult who has good rapport with a reluctant reader can become an unobtrusive "partner reader" with these special books and special students. Some of our boys, especially after being partnered through a 2 or 3 books about penguins, skateboarding, cheetah's, motorcycles, sharks, etc. on their independent level start to "warm up" to reading - kind of like a cold engine warming up on a frigid morning. Takes patience, takes committment to matching books to readers (INTEREST as well as readability), takes gentle positive persistence - and we may not reach all of our "I hate to read" -ers, but we are reaching some of them.
I also second the recommendation of the Cris Tovani book - and go to hear her if you have a chance. She remains in a classroom, so her presentations are always up to date and about the kids she's teaching NOW. They have gone through too many years of not enjoying reading. It is a tough road. If you haven't read the book I highly recommend it.
