My school has us progress monitor fluency using grade level pieces.  I think 
that's kind of hard on the kids who aren't reading at grade level.  In terms 
of guiding instruction, I find it more useful to look at fluency if the child 
is reading something that is instructional or independent for their actual 
reading level.  Then I can take a closer look at their reading 
behaviors/miscues/strategies, etc. and make plans for how best to help them.
