Have you read Carol Lyon's book, Teaching Struggling Readers?  It is a very 
powerful book.  I have taught 6th graders who couldn't read, and Lyon's book 
probably addresses some issues you are having.  Also, I don't think you can 
go wrong with teaching these kids basic reading strategies like in Fountas & 
Pinnell's book, Guided Reading:  Good First Teaching for All Children.  Dr. 
Marie Clay said that we "create" a lot of LD kids by not addressing their 
confusions about print when they are beginning readers, so I suspect that a 
good number of your 6th graders may have missed out on good first teaching.
I have been teaching a long time, and I don't think there are any kits or 
programs out there that provide what these struggling readers need, since 
they will flourish with teaching based on close observation of what they can 
do & then individual attention to help them get where they need to go.
