Actually, the namespace "http://www.w3.org/TR/REC-html40" doesn't mean
anything. (Although I think we do currently interpret it as XHTML for some
legacy reasons.)

Actually, it does, and it should.
Mikel: Could you point me to the example in question? We should be doing
this fine, unless there is something else in the file that I am missing.
