Another section which could be useful is a box where the user describes what action they have taken in informing the site's owners. Another checkbox "I have contacted the site" (and if yes, a form to say what you told them. This could be useful "ammunition" for the evangelists. Or do we just paste our emails into the reporting form?
I doubt most users would do that much; it's more likely something for the evangelists to do. Contacting the site is a Bugzilla mindset approach, while this tool is supposed to obviate the need for the Bugzilla mindset (which is rarer than the number of users who file bugs, incidentally).
