It's set in the Internet control panel, along with the association.
Unfortunately, it's hidden in an `Advanced Options' pane
<http://joshforman.com/Images/mapping.gif> which takes quite a bit of
work to get to, so browsers need to provide a more obvious UI for it, as
MSIE does <http://web.jet.es/catania/panexpl2.gif>. (The browser UI
reads from, and writes to, the OS settings.)

Annoyingly, Mozilla's UI for file associations has never worked -- the
only association it lists is text/html.
