This is definitely a profile problem.  I cleared out all the cruft in the profile 
directory (everything except cook*.txt, *.db, *.s, bookmarks.html, and prefs.js), 
and let Mozilla recreate everything it needed.  The two problems above are 
solved, and there is one extra bonus - middle clicking on a link now opens a new 
tab *without switching to it*.  This is what I always wanted, and I was quite 
surprised to see this new behavior.  I'm not sure which behavior is "correct" - but 
I'm glad it now works the way it does.  ;)
