I too can say "my frequently accessed bookmarks are at the top
of the list - and filed in appropriate nested folders". There
was *nothing* that involved "how to pull that off" -- I simply
worked with the entries in Mozilla's 'Manage Bookmarks' window.

Have not had to struggle with bookmarks for several months now.
When I am in a site I want to return to, I "save this bookmark".
The entry gets put at the bottom of the list. I then open
'Manage Bookmarks', expand the "folder" where I want to put that
entry, and use "drag&drop" (one of the few times I ever use
"drag&drop" - I prefer CLI) to move that entry to the position
I want. [The breakthrough came for me when I realized I *could*
drag entries to __empty__ (i.e, just-created) folders !]

Two minor glitches which don't disturb me: If the cursor in the
"drag&drop" operation __hovers__ over an unexpanded folder, that
folder will sometimes *expand* even though I do not want it to.
The bypass is to "drag&drop" without hesitation (i.e., *don't*
hover). The other one I haven't noticed if it is still there or
not - it used to be that if the *last* line in the list was
affected, the "drag&drop" operation would complete successfully,
but the 'Manage Bookmarks' window would *not* properly show the
result. My bypass is to *not* affect the last entry in the list.
or if I do, then I immediately close the bookmarks window (if I
need to do more editing, I open 'Manage Bookmarks' again).
