[MP3 ENCODER] LAME even faster

I wouldn't have thought it was possible, but Chris' patches eek out
even more speed from LAME!

I'm now up to playing_time/encoding_time = .75 on my PII 266.  
These improvements will be in 3.04.  There are also a bunch of bug fixes
in the frame analyzer.  So if you notice problems like the input pcm
data doesn't match the output pcm data, they should be fixed in 3.04.
