Create a query with a calculated field say "FullName", which is =[lastname] &", " &[firstName]
If you have a field for AKA, just in case you have several John Smith's, you could add to the above after firstName the following: "( " & [AKA] &" )" Use this query & field FullName as the source for your combobox.
