The name of the form is Form Main Query.  The two controls (I assume you
mean the names of the boxes that come up) are A1 and B1.  That's A followed
by the number 1.  SQL looks like:
SELECT Main_Tbl[A1], Main_Tbl[B1]
FROM Main_Tbl
WHERE (((Main_Tbl[A1]) Like [Enter A1 Term: ]&"*") AND ((Main_Tbl[B1]) Like
[Enter B1 Term: ]&"*"));
