You actually need to patch/rebuild pcmcia-cs.

As it stands, I've only released the driver as a patch to pcmcia-cs. 
I'm working on a kernel patch.

Depending on the options your kernel was built with, you *may* need to
rebuild the kernel as well in order to disable the built-in pcmcia
modules.

You need to patch the pcmcia-cs sources with the gpr400_cs source. 
Rebuild and install pcmcia-cs and the driver should be ready to go.  See
the readme file in the gpr400_cs source for full instructions.  

You may need to reboot or at least restart pcmcia services in order for
the drivers to work.  Don't forget the device file.

You also need the gpr400 IFD.  Build and install the library.  add the
library to your readers.conf file and restart pcscd.
