I am fairly confident that this is not a physical problem, since I have now tested a GemPC USB-SL reader and card with identical results. In addition, running killall securityd as root (a shot in the dark) and then plugging in the reader allows the card to be read and used normally, but only the first time I do it - on subsequent tries the card fails, and I have to reboot the system to try again. I have also tried both readers and cards on a freshly re-installed mac, so I don't think it is anything idiosyncratic in my desktop's setup.
